Registration, questionnaires and documents for our suppliers.

Who can use these. Each form asks for your e-mail address and sends you a single-use code. Only addresses registered by One Blood UK can open them. If one will not let you in, contact us and we will check your registration.

How this page works

Start with the registration questionnaireIt is how a supplier enters our system. Nothing can be ordered from a supplier who is not registered and assessed.
Open it and verify your e-mailThe form asks for your e-mail and sends a six-digit code to that inbox. Only addresses OneBlood UK has registered will open it.
Answer every questionYour answers are read and scored. An incomplete questionnaire delays the assessment; it does not speed it up.
Something wrong on our side?Use the Complaint Report. It goes to the quality system, not to a mailbox somebody may be away from.

If a form will not let you in, it means the e-mail address is not on our list, or you typed a different one. Contact One Blood UK and we will check it — do not keep asking for codes.
